As a vital paraprofessional member of our home care team, you will work under the supervision of a Registered Nurse or Therapist to provide essential personal care and support to our patients. Your role is more than just a job; it is a calling that directly impacts the quality of life for those we serve. You will be the eyes and ears for our clinical team, providing crucial observations and documentation that ensure quality, continuity, and safety in each patient’s care plan.

Your Essential Roles and Responsibilities:
In this role, you will be the cornerstone of our patients’ daily comfort and well-being. Your responsibilities will include, but are not limited to:
Direct Personal Care:

Providing compassionate hands-on care including baths, back rubs, oral hygiene, and shampoos.

Assisting patients with dressing, undressing, and toileting activities to preserve their dignity.

Providing diligent skin care to prevent breakdown and ensure patient comfort.
-
Changing bed linens and assisting in keeping the patient’s living area clean and orderly.
Health and Nutritional Support:
-
Planning, preparing, and serving nutritious meals according to patient dietary needs.

Assisting with feeding, if necessary, to ensure proper nutrition and hydration.

Taking and accurately recording vital signs, including temperature, pulse, respiration, and blood pressure, when ordered and upon demonstration of competency.
Mobility and Therapeutic Support:
-
Safely assisting patients with ambulation and exercise as outlined in their individual plan of care.

Performing simple range of motion exercises and other basic procedures as an extension of therapy services, upon demonstration of competency.
Safety and Observation:
-
Meeting the safety needs of patients and using all equipment (such as foot stools and side rails) safely and properly.

Being a vigilant observer; reporting any changes in the patient’s condition or needs to the assigned supervising nurse immediately.

Providing reassurance and companionship, addressing the emotional and social needs of the patient.
Additional Duties:
-
Assisting patients with the self-administration of medication (preparing reminders, opening containers).

Performing patient’s laundry, as appropriate to the situation.

Adhering to all documentation procedures, care standards, and codes of professional conduct.
Who We Are Looking For: Qualifications & Attributes
Required Qualifications:
-
Successful completion of a state-approved Home Health Aide training and competency evaluation program, OR a state-approved competency evaluation alone, OR a state-approved Nurse Aide training program with current good standing on the state registry.

Must be at least 18 years of age.
-
Ability to read, write, and follow written instructions and document care provided.
-
Possess and maintain a current CPR certification.
-
Demonstrates empathy, tact, patience, and maintains excellent personal hygiene.

Is self-directing and able to work with little direct supervision while following a specific care plan.
Preferred Qualifications:
-
Proof of registration on the Home Care Aide Registry through the Department of Social Services, Home Care Services Bureau.

Licensed driver with an insured automobile in good working order (as required by the organization).
Important Job Limitations:
To ensure patient safety and adhere to strict healthcare guidelines, the Home Health Aide is expressly prohibited from performing duties that constitute the practice of nursing. This includes, but is not limited to:

Administering any medications.

Taking or transcribing physician’s orders.

Performing any sterile procedures or techniques that require the advanced training and skill of a licensed nurse.
