Key Responsibilities
• Clinical Care Management: Conduct comprehensive patient assessments, develop individualized care plans, and coordinate multidisciplinary healthcare services • Patient Education: Provide expert guidance on disease management, medication compliance, and health maintenance strategies • Documentation Excellence: Utilize WellSky Kinnser EMR and Andy AI technology for accurate, timely clinical documentation and OASIS assessments • Care Coordination: Collaborate effectively with physicians, specialists, and healthcare team members to ensure seamless patient care • Quality Assurance: Monitor patient progress, adjust care plans as needed, and ensure adherence to clinical standards and regulatory requirements • Family Support: Educate and support family members and caregivers in patient care techniques and safety protocols
Required Qualifications
Licensure & Certifications: • Valid Illinois Registered Nurse license • Current CPR certification • Valid driver’s license and auto insurance
Experience & Skills: • Open to new graduates and experienced nurses – comprehensive training provided • Willingness to learn WellSky Kinnser EMR system (training included) • Strong clinical assessment and critical thinking abilities • Outstanding written and verbal communication skills • Patient-centered care philosophy with cultural sensitivity • Strong organizational and time management capabilities • Passion for community service and home-based healthcare
